A Multi-disciplinary Approach to Reduce Readmissions

Learn how Southport & Ormskirk Hospitals NHS Trust have implemented a multi-disciplinary team within their emergency department to reduce readmissions.

As part of the multidisciplinary team within the emergency department, Southport & Ormskirk NHS Trust’s aim is to reduce delays and inefficiencies, prevent unnecessary admissions and enable timely discharge of patients to home or community settings. OT’s and Physio’s in A&E services have been successful in reducing the number of admissions and readmissions into hospital. With this in mind, they have set up a therapy service within the emergency department at Southport hospital. Within their emergency department, the roles of OT and Physio overlap, enabling them to become a ‘blended’ therapy team working closely together providing interdisciplinary assessments, thus streamlining patient care.
