Since February 2014, a clinical pharmacy service closer to home has been provided by pharmacists from Mid Yorkshire Hospitals NHS Trust. This evolved into a multidisciplinary team and a joint venture between Mid Yorkshire Hospitals NHS Trust, Wakefield Metropolitan District Council (WMDC), Age UK and Carers Wakefield. The medicines optimisation and pharmacy service Connecting Care team aims to deliver a clinical pharmacy service and implement the principles of medicines optimisation to patients in the community.

Objectives of the service include:

  • Carrying out clinical medication reviews
  • Providing patients with education, advice and support
  • Identify and minimise side-effects and adverse reactions from medication

You can read the full list of objectives, benefits, and two detailed case studies from the Connecting Care Team on the members’ area, under the Pharmacy & Medicines Optimisation project outputs.
