Learn how the Mid Yorkshire Hospitals NHS Trust created an integrated approach to reduce unnecessary hospital admission and delayed discharges.
Since February 2014, a clinical pharmacy service closer to home has been provided by pharmacists from Mid Yorkshire Hospitals NHS Trust. This evolved into a multidisciplinary team and a joint venture between Mid Yorkshire Hospitals NHS Trust, Wakefield Metropolitan District Council (WMDC), Age UK and Carers Wakefield. The medicines optimisation and pharmacy service Connecting Care team aims to deliver a clinical pharmacy service and implement the principles of medicines optimisation to patients in the community.
Objectives of the service include:
- Carrying out clinical medication reviews
- Providing patients with education, advice and support
- Identify and minimise side-effects and adverse reactions from medication
