With the Model ICB Blueprint and Fit for the Future plan setting a clear direction for integrated, data-driven care, the NHS Benchmarking Network is responding quickly to support ICBs in turning these ambitions into action.
This session is a discussion forum to provide practical support, insight, and networking opportunities to empower you to identify and deliver your Population Health Management (PHM) requirements. This includes:
Benchmarking data to inform strategic commissioning and investment decisions
Population health data and requirements for emerging tools
Insights to support the development of Neighbourhood Health Services and integrated care models and discussion around further developments
Support for aligning local delivery with national expectations on transparency, quality, and financial sustainability
It’s also a great opportunity to connect with peers tackling similar challenges across the country.
